National Core Indicators (NCI-AD) Survey
Rhode Island does a phone survey called the National Core Indicators – Aging and Disabilities (NCI-AD) survey. We are conducting the survey on behalf of the Executive Office of Health and Human Services (EOHHS). We want to know if the Medicaid-funded services you receive help you live the life you want.

Why does Rhode Island do this survey?
The answers we get from you and others in Rhode Island who take the survey let us know if the services you receive are helping you to have a good life. The survey also gives us information we can use to help improve services for older adults and individuals with physical disabilities living in Rhode Island.
How was I chosen?
The Sherlock Center randomly picks about 1,000 adults who receive services from the Executive Office of Health and Human Services (EOHHS) and asks them to take this survey with us.
What is the survey like?
- A Sherlock Center surveyor will call you to conduct the survey or schedule a time that works for you.
- You can have a family member or friend with you.
- We will provide an ASL interpreter, help in a language other than English or anything else you need to understand and answer the questions.
- The person doing the survey will ask you questions about your services, your job if you work, whether you feel you are treated fairly and know your rights, what plans are in place to support you, the activities you do outside your home, the decisions you make about your life, and your health and safety.
- Your answers to the survey questions are kept separate from your name and we will never share your name or personal information with anyone.
Why should I take this survey?
It is your decision whether to take the survey. If you do, your answers are put together with answers from others to help us understand how services help people live the life they want. Most importantly, we want to hear what you have to say about your supports and services because your voice matters.
