Employment and Day Activity Outcomes Survey

The 2024 Bi-Annual Survey link will open on November 1 and close on November 30. A Survey ID is required to start or update a survey. If you need a Survey ID for a person supported by your agency, contact your agency liaison.

If you are a self-directed support user, please use the link below to complete your survey. You will be asked to enter your Survey ID to open the survey. The Survey ID is listed in the packet sent to your home. For help, contact the Sherlock Center.
